26 May 2017 – The Newborn Hearing Screening Reference Center (NHSRC) of NIH requires ALL Certified Newborn Hearing Screening Centers to transmit data gathered from March 31, 2015 to March 31, 2016 using the standard spreadsheet template or MS Excel format on or before August 31, 2016 as indicated in the MOP Version 1.0 (DOH Circular 2014-0150 published March 31, 2014).
*To ensure data security, please fill out with your contact details below before downloading the template.
To comply with Version 1.0 of the Manual of Operations (MOP) of RA 9709 for Newborn Hearing Screening, all certified centers should regularly submit monthly reports to the NHSRC. Due to stringent processes, NHSRC made data encoding for March 31, 2015 to March 31, 2016 easier by providing a spreadsheet template to help newborn hearing screening centers summarize their screening transactions per month. This is to address backlogs in data transmission while registry cards are distributed and while the implementation is in its infancy.
Meanwhile, all certified newborn hearing screening centers are highly required to continue with the current online data encoding while data for March 31, 2014 to March 31, 2016 are encoded in the spreadsheet. Failure to comply is considered an offense in data transmission. After 3 identified offenses, the newborn hearing screening center shall be removed from the official list of Newborn Hearing Centers and will be required to undergo re-certification once removed from the list.
This requirement is further explained through an official communication letter below.
