DEPARTMENT MEMORANDUM NO. 2019-0410 STOP ONLINE ENCODING

The Department of Health (DOH), in partnership with the Newborn Hearing Screening Reference Center (NHSRC)-National Institute of Health (NIH), UP-Manila, suspended the use of the online registry database. Hence, all facilities are advised to 1) stop encoding their data in this platform and 2) use the updated excel formats starting September 1, 2019.

Furthermore, the NHSRC developed two (2) new excel format for reporting: 1) Newborn Hearing Screening Registry Excel Sheet, a hard copy of this report with complete data and results should be kept in the health facility and will be checked during monitoring and evaluation; 2) Monthly Summary Report, a summary of the hearing screening results which will be submitted on or before the 7th day of the succeeding month. All health facilities are also advised to submit an updated Monthly Report Summary from the first day the newborn hearing screening was performed up to the present on November 25, 2019.
